It ended up being the timing chain. Apparently it was one of the worst they've ever seen without actually wrecking the engine. It was a nightmare getting it sorted out and won't bore you with the facts but now it's fixed and sounds so much better.

Looks like a conventional timing belt, but a short simplex chain linking the two camshafts, if I've got the right engine.
